Well, the dictionary shows only the basic form of a word. However, when you put the word into a sentence, the ending changes.
The form depends on whether the word is considered as masculine, feminine or neutral and also on the words which lie in front and behind it.
A word 'dom' meaning ' house' can have 9 endings for singular form: dom, domu, domowi, domem, domy, domów, domom, domami, domach and 5 for plural form: domy, domów, domom, domami, domach
Also the verbs have different endings for every person. That's why we often omit the word I, you, he, she etc in the senteces as the ending already indicates it.For example: (ja) Mieszkam w Anglii. - (ty) Mieszkasz w Anglii.
There are many different endings, many rules, and even more exceptions to these rules. But don't worry, even Polish people make mistakes quite often. Learn the words in the full phrases and later you may notice common patterns :)
